How to Register
- Download and print the appropriate registration form for your program.
- Complete all sections including emergency contacts and medical information.
- All participants must also complete the Liability Waiver & Medical Form.
- Return completed forms to Barb Koster before your first session.

Summer Camp Registration Form
Summer CampRegister your child for summer camp (June 8 – August 1). Includes session selection, emergency contact, and medical information.
Lesson Program Enrollment
LessonsEnroll in our year-round group lesson program for children ages 7 and up. Beginner through advanced levels.
Saturday Program Registration
Saturday ProgramSign up for our Saturday Camp Program offered most Saturdays during the school year, 9am–3pm.
Liability Waiver & Release
RequiredRequired for all participants. Covers general liability, assumption of risk, and emergency medical authorization.
Medical Information Form
RequiredHealth history, allergies, medications, and emergency contact information for your child.
